TriMet “Honored Citizen” is how TriMet identifies seniors age 65 or older, people on Medicare and people with mental or physical disabilities. OHSU Partnership Project Offers Medical Case Management, Navigation for Immigrants and Refugees living with HIV/AIDS (Minority AIDS Initiative) and Prevention Counseling Services.
If you need the Kaiser or the County clinic, Leslie can put you in touch with the right people, and if you are going to get your medical care at any of the other clinics (Fanno Creek, Legacy, Oregon Clinic, OHSU, Providence, VA, etc.) a visit with Leslie is the gateway to getting case management and, if you need it, nurse case management and Trimet Tickets.
